The enchantment for the 2nd slot on Insignias (Accessories): You can choose the desired enchant and the enchantment won't fail. 
Costs: 30,000,000 zeny, 50   Curse Eroded Gemstone (25740)  , 200   Contaminated Magic (6755)  and 3   Powerful Dimensional Essence (7925)  (dropped in Treasure Room Old Glastheim).
